BIRTH AND DEATH CERTIFICATES

Summit County Public Health provides certified birth certificate copies for every person born in the State of Ohio and certified copies of death records for every person who died within Summit County.  The fee for all certified certificates is $22.00 per copy.  Checks and money orders should be made payable to Summit County Public Health.  Credit and debit cards are also accepted (additional fees apply). Orders by phone are also accepted. For 24/7 bi-lingual ordering and assistance please call 1-866-221-6684.  The fee for using this service is $9.95.

ATTENTION FUNERAL HOMES:

Our office now accepts death records for electronic filing. 
This means signed death certificates can be e-mailed (along with the request for approval form) to vitalscph@schd.org.  Our office will review the certificate and accept or reject the certificate for filing within one business day.  Once the certificate is accepted our office will send an e-mail with ordering instructions.
